4. Media Release Form
5. Prohibited Employment Declaration (for those over 18 years of age)
6. Once completed please email to registrar@blackheadslsc.com.au with your online payment receipt.

Nippers Fees 2021/2022
SLSA requires each nipper to have a club cap $15 and pink safety vest $25.
It is a requirement of the Club that each nipper has a parent or guardian join the club as an Associate.

Nipper & Juniors 5yrs – 15yrs (no Active Kids Voucher) $50
Nipper & Juniors 5yrs – 15yrs using Active Kids Voucher $100
(includes cap and pinky)
Assisting Member (assisting role within nippers) $20

Working With Children Check
All members aged 18 and over must submit to the club a Working With Children Check (WWCC). This is a best practice recommendation from SLSA.
